Insulin resistance means the body requires higher insulin levels to manage blood glucose, and elevated insulin promotes fat storage particularly in the abdominal region, drives hunger, and impairs the body’s ability to lose weight through standard caloric restriction alone.

In addition, elevated androgens (male hormones) in PCOS can alter fat distribution toward central adiposity, which is metabolically harmful and particularly difficult to lose. Disrupted sleep from PCOS related conditions and the psychological burden of the condition, including anxiety and depression, which are significantly more prevalent in women with PCOS, further impair weight management efforts.

What Role Can GLP-1 and dual GIP/GLP-1 Medication Play in PCOS?

GLP-1 and dual GIP/GLP-1 receptor agonists address several of the underlying mechanisms that make weight management difficult in PCOS. By improving insulin sensitivity and reducing fasting insulin levels, they address one of the core metabolic drivers. By reducing appetite and promoting satiety, they overcome some of the hormonally driven food seeking behaviour that makes caloric restriction difficult. By promoting preferential reduction in visceral adipose tissue, they target the most metabolically harmful fat depot.

Clinical trial subgroup analyses and smaller dedicated PCOS studies have shown that GLP-1 and dual GIP/GLP-1 therapy produces meaningful weight loss in women with PCOS and improves menstrual regularity, androgen levels, and insulin sensitivity. Semaglutide in particular has been assessed in PCOS populations with promising results, though none of these medicines are specifically licensed for PCOS as an indication.

Metformin and PCOS: An Important Context

Metformin is widely used in PCOS management, both for its insulin sensitising effects and for its modest weight neutral or weight reducing properties. For women with PCOS who have not already tried metformin, it is often a first line consideration.
